While there are some shemales who are born intersexed and identify as such, many shemales are transgender and choose to transition to the gender they identify with. The process of transitioning to become a shemale is often long and complicated, and it usually takes several years of hormone therapy, mental health treatment and sometimes surgeries to fully transition.
